"A supportive housing program serving homeless and disabled residents of Somerset, Wicomico, and Worcester counties. Provides housing to eligible participants through ongoing monthly rental assistance and case management to provide service coordination, assist them in obtaining benefits, and assist them obtaining services and treatment to address their disabilities. In 2013, the program provided 73 homes for homeless individuals and families to obtain permanent housing and case management services."
